Don't think I had any of those, my SAD didn't set in til I was a teenager and the teachers didn't write long reports. I did get a couple of short "needs to interact" or "more class participation" though.
In college when writing some sort of personal report I didn't mention SAD I just said something like "I feel everyone is watching and judging me" and the teacher wrote back "But they're not." Right...
